By completing the feedback survey you will be in with a chance of winning a £50 Slimfast Bundle. The first Five Hundred (500) respondents will recieve the £50 prize. Prizes are limited to one per household.

Survey Terms and Conditions

By entering the Survey, you agree to the following additional terms and conditions:

1. Entering your details at slimhappy.co.uk
2. The Survey is open to residents over 18 residing in England, Scotland or Wales, with the exception of our employees, and those of our associated companies and agents and their families and friends and is limited to one (1) entry per person. In entering the Survey, you confirm that you are eligible to do so and eligible to claim the Prize. We may require you to provide proof that you are eligible to enter the Survey.
3. Only complete and clearly legible entries will be eligible for the £50 prize.
4. The winners will be notified by email.
5. If there is any reason to believe that there has been a breach of these terms and conditions, we may, at our discretion, exclude you from participating in the Survey. We reserve the right to withdraw or amend this Survey, or these terms and conditions, at any time.
6. Unless stated otherwise in the Survey the Prize cannot be substituted for a cash alternative and the Prize cannot be transferred to a third party.
